You may pick-up the registration form at the district business office located at
3801 West Lake Ave, 3rd Fl, Glenview
or, you may follow the simple steps below:
1. PRINT OUT THE REGISTRATION FORM
(4 pages)
YOU MUST PRINT OUT ONE SET FOR EACH STUDENT
2. Complete Page 1
To find the appropriate bus route please click on the "Bus Routes Link" to the left
3. If paying by Mastercard or VISA, please fill out the credit card information at the bottom of Page 1
4. If paying by check or money order, please make payable to: Glenbrook HSD 225
5. Parent AND Student MUST SIGN and date the bottom of Page 3
6. Mail completed forms and payment to:
